Clients who are privately funded, NDIS plan, NDIS self managed or INCWA funded can be referred to our service. Referrals can be made by the client, support coordinator or health professional. Please fill out our referral form , email it to admin@evelynsimot.com and we will contact you to discuss the referral and make arrangements.
